About the role

We’re looking for a compassionate and motivated Community Care Worker to join our dedicated Adult Social Care team at the London Borough of Hillingdon.

In this rewarding frontline role, you will assess and identify the needs of adults and older people, developing and reviewing care plans that promote independence, dignity, and wellbeing. You’ll work closely with residents, carers, and partner agencies to ensure services remain person-centred, effective, and responsive to changing needs.

Key responsibilities
  • Carry out needs assessments and identify eligible care and support needs in line with statutory requirements.
  • Develop, implement, and review person-centred care plans that promote independence and quality of life.
  • Work with individuals, families, and carers to achieve positive outcomes and support rehabilitation and recovery goals.
  • Ensure that vulnerable adults are safeguarded from abuse and neglect, taking appropriate action where necessary.
  • Collaborate with health professionals, voluntary organisations, and other council teams to deliver joined-up care.
  • Review existing care packages to ensure they remain appropriate, proportionate, and cost-effective.
  • Maintain accurate and timely records in accordance with professional and legislative standards.
About you

You’ll be a caring, proactive individual who’s passionate about improving lives and promoting independence. You’ll have excellent communication skills and the ability to work sensitively with residents, carers, and professionals.

You should also have:

  • Experience working within adult social care, health, or community settings.
