Overview

Due to our expansion in our Clinical Services we are seeking to appoint an enthusiastic and experienced Adult Community Nurse. This is a new role in the area and an opportunity to create and establish our Clinical services in the area. You will work with Carers and complex care patients in their own homes within the community, ensuring that Carers are suitably trained and competent to deliver the care needed. You will assess the risks and needs of the patient to devise an individual holistic, detailed electronic care plan and ensure it is regularly evaluated so that patients receive supervised evidence-based nursing care in accordance with local contracts.

Responsibilities
  • Manage your own caseload and diary, typically working Monday to Friday 09:00 – 17:00.
  • Assess patient risks and care needs and develop individual holistic electronic care plans; regularly evaluate plans to ensure evidence-based care.
  • Work with Carers and complex care patients in their own homes within the community and ensure Carers are trained and competent to deliver the care required.
  • Submit clinical reports and updates to CCGs regarding patient packages as per contractual agreements.
  • Maintain clinical updates and evidence of PREP (Post Registration Educational Portfolio).
  • Demonstrate leadership skills/experience where applicable and train clinical and non-clinical staff as needed.
  • Hold current registration to practice.
  • Driver's licence and use of a vehicle for work.
Qualifications
  • Clinical experience including tracheostomies, ventilators, stoma care, etc.
  • Ideally, previous knowledge and/or experience in working in a complex care setting or community setting.
  • Excellent communication skills and the ability to train clinical and non-clinical staff.
  • Ideally have leadership skills/experience.
  • Current registration to practice.
