Job description

The Clinical Lead will support the Operational Team Leads across both Newtown and Welshpool CMHTs, leading a multidisciplinary team in their deployment in the delivery of a clinically effective Nursing Service, whilst providing assessment, treatment and Care Co-ordination to patients in the local community who are referred to the Community Mental Health Team.

Responsible for the pro‑active case management of a wide range of patients many of whom have complex needs.

Accountable to the Health Board Nurse Director and the Nursing and Midwifery Council (NMC) to provide safe, high‑quality care to patients by implementation of policies/ standards and guidance produced both national and locally. These will include from NMC, Welsh Government, Powys Teaching Health Board, and others.

The person appointed will be expected to undertake a range of duties to include:

  • Being responsible and accountable for an identified case load, prescribing, providing supervision to the team, liaising with partner agencies, dealing with concerns, chairing and engaging in MDT meetings, monitoring standards of practice within the team, recruitment and appointment of staff, absence management and the deployment and allocation of duties.

Consistently work with the team to ensure standards of patient care are high and maintain safety and quality of the patient experience.

Within the team, lead the implementation of and monitor local and national nursing policy, procedures, and guidelines to support best practice and ensure they are adhered to together with current legislation.

Using expert knowledge of managing and delivering care for patients with highly complex and unstable health needs within the community setting and ensure the delivery is safe, efficient, and effective.

Responsible for the accurate recording, timely collection, completion, and submission of relevant information to support patient care, operational management, nursing staff and skill mix, service development and audit, analysing and interpreting this information to inform leadership within your area of responsibility.

Undertake a training needs analysis for nursing within your team, and implement agreed training plans.

Actively participate in the research and development agenda.
