Job description

The Harefield Hospital, part of Guy's and St Thomas' NHS Foundation Trust, is the largest postgraduate specialist heart and lung centre in the United Kingdom. We are seeking to appoint a Junior Clinical Fellow in Transplant Medicine for an initial 6‑month period, with a possible extension of up to 2 years by mutual agreement.

The successful candidate will undertake the admission and day‑to‑day care of cardiology and respiratory patients, including short‑term investigations and longer‑term treatment. They will participate in an on‑call rota and will be expected to manage emergency cases admitted daily.

Clinical Responsibilities
  • Ensure good communication within the Transplant Service and collaborate with medical, surgical colleagues, and other departments.
  • Take history, examine, and record all findings of patients admitted under the Transplant Service, including those in the Intensive Care Unit.
  • Assist in out‑patient sessions when required.
  • Arrange transfer of patients to/from other hospitals or discharge home, and prepare transfer/discharge summaries.
  • Perform minor surgical procedures such as chest aspiration, chest drain insertion, central venous line insertion, and arterial blood gas sampling under supervision of a Specialist Registrar if necessary.
  • Dictate/input urgent discharge summaries and ensure they are forwarded to the patient’s GP or receiving hospital as soon as possible.
  • Supervise and train junior staff allocated to the Transplant Service.
  • Work as a full member of the multi‑disciplinary team, including presenting at weekly MDS meetings and other division and Trust meetings.
  • Undertake regular ward rounds to ensure effective management during the in‑patient stay.
  • Keep the Ward Sister, Specialist Registrar and Consultant informed of any patient who is causing concern.
  • Communicate progress to patients’ relatives.
  • Participate in the Medical Audit Programme and update relevant patient database programmes.
  • Participate fully in the service and training programme of the Transplant Service and Heart Division.
  • Respond to occasional emergencies and unforeseen circumstances that may arise.

  • Maintain current knowledge of cardiology, heart failure, respiratory medicine, and transplantation, and participate in a recognised programme of continuing medical education and professional development.
