Job description

Job title: Spinal Injury Specialist Case Manager

Location: UK-wide (home based but with regional travel)

Salary: £40-55k

Status: Full-time or part-time available (min 3 days per week). Zero hours also considered.

Purpose of Role: To assess and understand an injured person’s specific needs and use your clinical and professional skills to ensure a maximal recovery. This extends beyond the individual’s presenting physical and functional abilities, and encompasses all aspects of their social and vocational situations and life participation.

Key Responsibilities:

  • To proactively manage a caseload of rehabilitation clients or clients requiring complex care from referral and assessment through active case management to case closure within an agreed geographical location.
  • Instructing, coordinating and progressing multi-disciplinary intervention with your client as the central focus
  • Working alongside other invested stakeholders, such as solicitors and insurers, allowing access to funding for private and augmentative intervention that may not be possible within standard statutory service provision
  • Caseload numbers vary depending on case complexities.
  • To be responsible for assessing and developing rehabilitation or care packages for clients with stakeholder agreement or funding.
  • To be responsible for liaising and maintaining regular communication with the rehabilitation client, case stakeholders and clinicians, keeping all case records accurate and up to date.
  • To accurately request, agree and manage funds allocated for rehabilitation or care on each case, reviewing and processing treatment invoices and monitoring overall case spend.

Person Specification:

Experience

Essential:

Minimum of five years of clinical experience in the field of neurological and complex orthopaedic rehabilitation including spinal cord injury.

A basic understanding of the funding of rehabilitation or complex care following injury including CHC funding, private funding, Insurer funding or post settlement funding agreements.

An understanding of GDPR, Safeguarding and Capacity.

Desirable:

Prior case management experience within the NHS and/or personal Injury industry.

Experience or understanding of the rehabilitation and care requirements and local provision for those suffering from complex injuries.

An understanding of statutory services provisions in relation to life-changing injuries.

Skills & Knowledge

An awareness and understanding of NICE guidelines in reference to neurological injury to allow for thorough assessment and rehabilitation coordination and planning including MDT collaboration.

An understanding of The Rehabilitation Code.

An awareness and understanding of NICE guidelines in reference to neurological injury to allow for thorough assessment and rehabilitation coordination and planning including MDT collaboration.

Attributes

Excellent written skills: compilation of extensive reports, attention to detail, professional e-mail communication

Ability to assess holistically and compile clinically sound recommendations for intervention

Ability to direct and coordinate a multitude of different services to ensure effective outcomes

Ability to convince and influence stakeholders through sound clinical judgement and reasoning

Willingness to travel to hospital, residential and clinic locations by your own means or via public transport

Qualifications and Training

Qualification in nursing, physiotherapy, occupational therapy or other clinical qualification.

Active HCPC registration, professional board membership and public liability insurance
