Care Team Nurse (LPN/RN)
Locations: Total Health Community Clinic (Monmouth) | West Salem Clinic (Salem)
Job Status: Full-Time | Monday – Friday | 8am – 5pm with optional 4/10 work schedule
We are currently seeking a compassionate, team-oriented Care Team Nurse (LPN/RN) to support patients across two clinic locations.
Provide practice management support to the clinical team, embracing the Patient Centered Primary Care Home (PCPCH) by recognizing the patient as the central focus. The Care Team Nurse, in collaboration with other health care professionals, addresses patients’ wellness, acute illness, chronic disease, disability, and end-of-life needs. Practice includes provision of telephone advice to patients with minor acute problems; patient education in areas related to self-care; support of the ongoing management of patients with stable chronic disease; monitor of acutely ill patients during the routine office visits; and performance of a variety of procedures that are within the scope of the professional nursing practice. The Care Team Nurse oversees the respective provider’s assigned patient panel by directing and facilitating care to support the patient, incorporating the nursing process: assessment, planning, intervention, and evaluation.
