Job summary

Porthaven Care Homes in High Wycombe is seeking an experienced Care Team Leader for Chiltern Grange Care Home. You will lead a team of Carers and Senior Carers to deliver person-centred care and maintain residents' dignity.

The role requires NVQ Level 3 in Health and Social Care, medication administration, care planning, and strong leadership. Night shifts (8pm–8am) with alternate weekends and a DBS check to be completed.

Qualifications

• You are willing to work towards or have your NVQ Level 3 Diploma in Health and Social Care or equivalent.
• You have previous experience in a care home environment, ideally in a Senior Carer role.
• You are confident in administering medication within a care home setting, including supporting multiple residents.

Responsibilities

• Leading and inspiring a team of Carers and Senior Carers in the day to day running of the home.
• Communicating regularly with residents' families and Health care professionals to write and adapt care plans.
• Administering medication and completing risk assessments.
• Ensuring the needs of the residents are always met.

Leading a team of Carers and Senior Carers, you will deliver person-centred care to our residents to ensure they retain their dignity and individuality. By leading a happy and competent team, you can really make a difference in your home and also the local community. Key responsibilities include:

• Leading and inspiring a team of Carers and Senior Carers in the day to day running of the home.
• Communicating regularly with our residents' families and Health care professionals to write and adapt care plans.
• Administering medication and completing risk assessments
• Ensuring the needs of the residents are always met.
• Ensuring residents daily records are up to date and accurately reflect the care provided.

The successful candidate will be subject to the provision of satisfactory references and a Disclosure and Barring Service (DBS) check, and they must be authorised to work in the United Kingdom.
