Job summary

A healthcare practice in Purbrook is seeking a Care Navigator to join their team. The Care Navigator will help patients navigate the healthcare system, acting as the first point of contact for inquiries. The role requires excellent interpersonal skills, attention to detail, and the ability to work in a fast-paced environment. Responsibilities include assisting patients with appointments, managing queries, and ensuring high-quality service delivery. The position requires 31 hours weekly, including 1-2 Saturday shifts per year.

Qualifications

• Experience within a customer-facing role.
• Experience of using IT systems in a work environment.
• Experience working in Primary Care or similar.

Responsibilities

• Assist and direct patients to appropriate services.
• Respond courteously to patient queries.
• Support appointment access via digital triage forms.
• Maintain patient confidentiality.
