Introduction

We are seeking a motivated and compassionate Care Navigator to join our team. The Care Navigator plays a key role in supporting patients to access the right care, at the right time, by providing information, guidance and signposting to appropriate services across health, social care and the voluntary sector. Working closely with clinicians and the wider multidisciplinary team, the post holder will help improve patient experience, reduce health inequalities and support people to manage their health and well being more effectively.

Main duties of the job

To be responsible for undertaking a wide range of duties and the provision of general support to the rest of the team. Duties can include but are not limited to: greeting and directing patients, effective use of the appointment system, booking appointments, scanning, processing patient registrations and deductions and assisting patients as required; liaising with multidisciplinary team members and external agencies such as secondary care and community service providers; and providing effective, high quality and responsive care appropriate and timely within a safe environment to meet the needs of our registered patients without restriction or discrimination, adhering to information governance regulations at all times. You need to be adaptable to the changing needs of the NHS and our patients.

Working Hours

Monday 8:30am - 6:00pm

Tuesday 8:30am - 6:00pm

Friday 8:30am - 6:00pm

Every other Sunday - 8:00am - 12:30pm

Experience and skills

Office experience, practical experience of working with others, experience of using own initiative, experience of customer service, experience of working within a general practice reception environment, and practical experience of computerised recording systems.

Qualifications Essential criteria
  • GCSE grade A to C in English and Maths
  • NVQ in Customer Service
