Job summary

Care Outlook in London and the South East seeks a dedicated Care Coordinator to lead rosters, supervise staff, and develop care plans for service users in East Sussex, Hastings. The role focuses on delivering high-quality, person-centred care within a supportive team.

You will ensure assessments meet best practice, work with the recruitment team to maintain staffing levels, handle emergency cover, and uphold CQC standards. A driving license and access to a vehicle are preferred.

Qualifications

• Knowledge of CQC standards and CQC compliance (essential).
• Experience rostering care calls on an electronic system.
• Experience in Care Coordinating or 2 years in Health & Social Care (preferred).
• Full driving license and access to a vehicle (preferred).
• NVQ Level 3 Health & Social Care (preferred).

Responsibilities

• Roster and template care calls using an electronic system.
• Direct management of the care team via supervisions, appraisals, and training.
• Create, manage, and monitor care plans to meet service users' needs.
• Ensure care assessments and risk planning meet best practices and user aspirations.
• Collaborate with Recruitment to maintain staffing levels.
• Provide care services and arrange emergency cover as needed.
• Ensure telephones are answered promptly and communications are courteous.
• Work with Branch Manager to address quality standards, complaints and feedback.
• Participate in the paid on-call rota and uphold CQC regulations.
