Job summary

Care Outlook is expanding in London and the South East of England. We are recruiting a Care Coordinator to maintain and develop our extra care service Ensham House.

You will roster and template care calls, manage the care team, and ensure staff rosters meet service users’ needs. The role requires knowledge of CQC standards, experience with rostering on electronic systems, and flexibility to cover on-call duties.

Qualifications

• Knowledge of CQC standards and compliance (essential).
• Experience rostering care calls on an electronic system.
• Flexibility to cover on-call shifts (essential).
• Experience in Care Coordinating or 2+ years in Health & Social Care (preferred).
• NVQ Level 3 Health & Social Care (preferred).

Responsibilities

• Roster and template care calls using an electronic system.
• Direct management of the care team with supervision, appraisals and training.
• Create, manage and monitor customer care plans and staff rotas.
• Ensure care assessments and risk planning meet best practice and service user aspirations.
• Coordinate with Recruitment to ensure staffing levels.
• Provide care services during emergencies and arrange cover as needed.
• Ensure telephone contacts are managed professionally and courteously.
• Collaborate with the Branch Manager to address quality standards, complaints and feedback.
• Participate in paid on-call rota and support CQC compliance.

We are currently recruiting for a Care Coordinator to maintain and develop our extra care service Ensham House.

Core Duties & Responsibilities as Care Coordinator:

• Primary role is to Roster and Template care calls using an electronic system
• Direct Management of the care team monitoring staff development through supervisions, appraisals, and training to assist them in delivering a high-quality service
• Creating, managing, and monitoring customer care plans, ensuring staff rotas meet service users' needs and aspirations
• Ensuring care assessments, care and risk planning meets best practice and customers aspirations
• Work with Recruitment Team to ensure sufficient current and future staffing levels
• To provide care services to service users in an emergency, arrange emergency cover when required and cover all/ any calls reasonable where required
• To ensure that the telephones are answered promptly, and people are spoken to in a polite and respectful manner
• To work alongside the Branch Manager ensuring that all quality standards, complaints, and comments are dealt with promptly
• Participate in the paid on-call rota
• Put in place measure to comply with CQC regulations and work to achieve the best care standards possible

As a Care Coordinator, you will be offered the following:
