Job summary

Care Outlook Ltd is seeking a Care Coordinator to join our expanding home care team covering London and the South East. The role focuses on rostering, supervising care staff, and developing care plans to meet service users' needs and aspirations.

You will ensure assessments and risk plans align with best practice, support recruitment, manage emergencies, respond to calls promptly, and uphold CQC standards. A driving license is preferred and remuneration ranges from £27,000 to £29,000 plus

Qualifications

• Experience rostering care calls on an electronic system.
• Experience in Care Coordinating or 2 years in Health & Social Care (preferred).
• Full driving license and access to a vehicle (preferred).
• Knowledge of CQC standards and compliance (essential).

Responsibilities

• Roster and template care calls using an electronic system.
• Manage and monitor the care team through supervision and training.
• Create and monitor care plans to meet service users' needs.
• Ensure care assessments and risk plans meet best practice and user aspirations.
• Coordinate with Recruitment to ensure staffing levels.
• Provide care services in emergencies and arrange cover as required.
• Answer calls promptly and maintain polite communication.
• Work with Branch Manager to address quality standards, complaints, and comments.
• Participate in paid on-call rota.
• Ensure compliance with CQC regulations and strive for best care standards.

Core Duties & Responsibilities as Care Coordinator:

• Primary role is to Roster and Template care calls using an electronic system
• Direct Management of the care team monitoring staff development through supervisions, appraisals, and training to assist them in delivering a high-quality service
• Creating, managing, and monitoring customer care plans, ensuring staff rotas meet service users' needs and aspirations
• Ensuring care assessments, care and risk planning meets best practice and customers aspirations
• Work with Recruitment Team to ensure sufficient current and future staffing levels
• To provide care services to service users in an emergency, arrange emergency cover when required and cover all/ any calls reasonable where required
• To ensure that the telephones are answered promptly, and people are spoken to in a polite and respectful manner
• To work alongside the Branch Manager ensuring that all quality standards, complaints, and comments are dealt with promptly
• Participate in the paid on-call rota
• Put in place measure to comply with CQC regulations and work to achieve the best care standards possible
