Salary is £25,364, with Monday to Friday 9am–5pm and on-call duties on a rota. A valid driver’s licence and access to your own vehicle are required for field-based travel.

Qualifications

• Experience & Organisation: efficient, organized Care Coordinator focused on community care.
• Stepping Up: healthcare experience helpful; knowledge of CQC, care plans and rotas
• NVQ level 3 is desirable

Responsibilities

• Manage care workers and field-based duties to ensure high-quality care.
• Assess and review individual care plans and supervise staff in the field.
• Coordinate rotas and maintain strong relationships with care workers; travel involved.
