Job Description

We are seeking a dedicated and compassionate Care Coordinator to join our dynamic team in Hayling Island, United Kingdom. In this fast-paced role, you'll be at the heart of delivering meaningful care that genuinely transforms our clients' lives. You'll work collaboratively across a busy, purposeful environment where your decisions and actions directly impact the wellbeing of the people we support.

**Key Responsibilities:** - Coordinate and optimise care plans in real-time, working closely with families, care staff, and healthcare providers to ensure clients receive seamless, person-centred support - Proactively monitor and adjust care plans to deliver optimal client outcomes, taking ownership of continuous improvement

  • Maintain meticulous, up-to-date client records and care documentation, ensuring accuracy and compliance at all times - Collaborate dynamically with the Care Manager and wider team to identify and address emerging client needs and concerns - Respond swiftly to schedule changes and communicate effectively with all relevant parties, keeping the care delivery running smoothly
  • Manage administrative tasks with precision, including data entry and report generation, supporting the broader care mission - Provide professional and friendly telephone support, often being the first point of contact for clients and families - Contribute to the On Call rota as required, ensuring continuity of care during critical moments
  • Champion compliance with healthcare regulations and company policies, safeguarding the people we serve - Actively participate in team meetings and drive improvements to care delivery processes, growing your expertise alongside the team - Coordinate with local healthcare providers and community resources, building relationships that enhance client outcomes
Qualifications
  • **Exceptional problem-solving abilities**: Demonstrated capacity to analyse complex situations, identify practical solutions, and make sound decisions in real-time, particularly in dynamic and demanding environments - **Strong interpersonal and collaborative skills**: Proven ability to build effective relationships with clients, families, healthcare professionals, and colleagues; excellent communication skills with the capacity to interact with empathy and professionalism across all levels - **Proven ability to thrive under pressure**: Experience managing multiple competing priorities, responding calmly to challenging situations, and maintaining composure whilst delivering high-quality care coordination
  • Previous experience in care coordination, scheduling, or healthcare administration, preferably in a fast-paced care setting - Proficiency in scheduling software and Microsoft Office programmes - Excellent organisational and time management skills, with the ability to multitask and prioritise effectively - Minimum of 5 GCSEs, including Maths and English, or equivalent qualifications
  • Keen attention to detail and accuracy in record-keeping and documentation - Knowledge of healthcare regulations and standards - Valid driving licence and access to ownvehicle, as local travel will be required - Willingness to undergo a DBS check and any required training - Experience in office administration is desirable
