Job summary

Home Instead in Gloucester is seeking a Care Co-ordinator with experience in domiciliary care to manage care packages and drive person-centered support. You will oversee the Client Experience team, conduct assessments, and ensure accurate care planning. This role requires local travel and on-call flexibility.

The ideal candidate will have related domiciliary care experience, be proficient in MS Office, and hold at least 5 GCSEs including Maths and English. DBS checks and references will apply.

Qualifications

  • Related experience in domiciliary care as Assistant Care Manager, Scheduler or Care Co-ordinator.
  • Minimum 5 GCSEs including Maths and English.
  • Good understanding of Microsoft Office programs (Word, Excel, Outlook).

Responsibilities

  • Manage care packages and day-to-day client care responsibilities.
  • Provide line management to the Client Experience team.
  • Complete client assessments and all new client paperwork.
  • Create and maintain visit schedules and care plans; assist with digital care planning.
  • Identify training needs and audit activity; maintain client and staff records.
  • Carry out client reviews and maintain regular contact with clients and carers.
  • Participate in on-call duties and be flexible to travel as required.
  • Support local community networking and marketing activities.

The Role:

- We arelooking for a detail-oriented Care coordinator with the ability to assist with the day-to-day responsibilities of the management of the Care packages and someone with the potential togrow with the organisation,

  • Management of clients and responsibility for their ongoing care and support with a focus on person centred care.
  • Provide line management to the Client Experience team, building and maintaining a positive team ethos.
  • Undertake consultation to new clients, complete assessments and complete all new client’s paperwork as required.
  • Create and maintain visit schedules as per client needs
  • Create, update and audit client care plans and assist with digital care planning.
  • Identifying training needs/follow ups in respect of auditing activity.
  • Maintain accurate client and Care Professional records on care management systems
  • Conduct client and Care Professional introductions.
  • Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.
  • Carry out client reviews and action accordingly.
  • Maintain regular contact with clients and their Care Professionals.
  • Any other duties as directed by your line manager.
  • Participate in on-call duties as required.
  • Ensure compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
  • Carry out any other duties deemed necessary for the successful operation of the business
  • Have a mindset ofcost saving in all actions
  • Be flexible to meet the demands of the business including participating in an on-call rota. Which means it may require you to support team members out in the field in emergency situations including undertaking Care visits as and when required.
  • Networking in the local community and supporting marketing activities.

Related experience in a Domiciliary Care as Assistant Care manager, Scheduler, or Care Co-ordinator is a must have

- Good understanding of all Microsoft Office programs (i.e. Word, Excel, Outlook etc.)
