Job summary

Home Instead Crawley is seeking a proactive Care Coordinator to liaise between clients, care teams and GP practices, ensuring timely care delivery and efficient rotas. You will build strong relationships with clients and carers, coordinating schedules and maintaining accurate records.

The role requires excellent communication, IT proficiency, and a driving licence. Prior domiciliary care experience and knowledge of care regulations are preferred, with opportunities for professional development

Qualifications

  • Excellent communication skills with the ability to build rapport quickly.
  • Proficiency in IT systems, including Microsoft Office and virtual communication platforms.
  • Experience in the domiciliary care sector delivering personal care services.
  • Knowledge of Health and Social Care legislation and regulations.
  • Strong organisational and time management skills.
  • Ability to learn and adapt to new technologies and software quickly.
  • Experience coordinating rotas or schedules.
  • Familiarity with CRM systems, particularly People Planner.
  • Full driving licence and access to a vehicle for travel within the territory.
  • Flexibility to meet changing business needs.

Responsibilities

  • Liaise between clients, Care Professional teams, and GP practices to maintain communication.
  • Prepare and maintain rotas, accounting for travel time, holidays, and training.
  • Develop and maintain relationships with clients and Care Professionals to ensure positive experiences.
  • Collaborate with the recruitment manager to ensure staffing levels are met.
  • Resource and schedule new and existing care packages promptly.
  • Maintain accurate client and Care Professional information in scheduling systems.
  • Ensure care plans and documentation comply with regulatory and internal audit standards.
  • Regularly audit electronic care notes and address concerns promptly.
  • Contribute to continuous improvement of care services and processes.
  • Coordinate and manage care schedules to meet client needs.

Job Description
  • Liaise between clients, Care Professional teams, and third-party entities such as GP practices to maintain clear communication and address care needs promptly.
  • Prepare and maintain rotas, taking into account travel time, holidays, training, and last-minute changes.
  • Develop and maintain excellent relationships with both clients and Care Professionals to ensure positive experiences.
  • Collaborate with the recruitment manager to ensure sufficient staffing levels are met.
  • Work closely with the Carepro's to resource and schedule new and existing care packages in a timely manner.
  • Maintain accurate client and Care Professional information in electronic scheduling and monitoring systems.
  • Ensure all care plans and documentation comply with regulatory and internal audit standards.
  • Regularly audit electronic care notes and address any concerns regarding care needs promptly.
  • Contribute to the continuous improvement of care services and operational processes.
  • Coordinate and manage care schedules, ensuring efficient allocation of Care Professionals to meet client needs
Qualifications
  • Excellent communication skills with the ability to build rapport quickly (Mandatory)
  • Proficiency in IT systems, including Microsoft Office and virtual communication platforms (Mandatory)
  • Prior working knowledge in care management applications such as Access Care Planning or Birdie (preferred)
  • Prior working knowledge in care management applications such as People Planner or IQ Timecards (preferred)
  • Level 3 NVQ in Health and Social Care or equivalent qualification (preferred)
  • Experience in the domiliary care sector, delivering a range of personal care services
  • Knowledge of legislation and regulations specific to Health and Social Care
  • Strong organisational and time management skills
  • Ability to learn and adapt to new technologies and software quickly
  • Experience in coordinating rotas or schedules (desirable)
  • Familiarity with CRM systems, particularly People Planner (preferred)
  • Problem-solving skills and the ability to work under pressure
  • Empathetic approach with a genuine passion for delivering high-quality care
  • Full driving licence and access to a vehicle for travel within the territory
  • Flexibility to meet the changing needs of the business
  • Ability to work collaboratively in a team environment
  • Keen attention to detail and commitment to maintaining accurate records
