Job description

You would be responsible for a defined caseload of individuals experiencing first episode psychosis as a care coordinator within the team, as such you would work with individuals to meet their identified needs using outcome measures, a recovery approach and facilitate discharge from secondary mental health services, or referral on as appropriate. Additionally the post holder would contribute to duty and clinic work and participate in team development issues.

Sponsorship Information

Once Checks Are Complete

The Recruitment Team will coordinate with you and the hiring manager to confirm a start date. You'll be booked onto a Trust Welcome Session and receive your Pennine Care NHS Terms and Conditions.
