A leading NHS Trust in Wakefield is seeking a dedicated support staff member for the Palliative Care Team. The role includes coordinating referrals, managing patient bookings, and assisting in outpatient clinics. The post holder will support and assist the Palliative careteam with the day-to-day management of patients that are referred to the SPCT.The post holder will support the CNS to triage referrals, booking patients into the CNS diary, making direct contact with the patient and signposting whereappropriate.
he post holder will support and coordinate the running ofoutpatient clinics, booking clinic appointments. The post holder will enableand assist the CNS team in delivering high standards of care to all patientsthat are referred to the SPCT who have a palliative care diagnosis and maypotentially be in their last year of life, they will support with low levelearly introduction to service visits. Part of the role will also be supportingthe team by managing bookings throughout the trust for EoL mandatory training,support training where required.
Assist in the coordination of referrals to the CNS team andtriage to appropriate team member
Triaging incoming telephone calls, managing onward referralsand sign posting to appropriate person or service.
To be available to patients, GP practices and Acute serviceteams and act as a resource for the SPCT
Coordinate outpatient clinics, which will include contactingpatients, booking appointments and then typing up clinic letters.
Assisting the CNS team with MDT actions as appropriate.
Input information onto relevant computer systems to ensureeffective communication between all individuals involved in the patient pathwayin a timely manner.
Develop and use appropriate communication skills to supportcontact with patients either on the telephone
Undertake routine phone calls to patients with the supportof the CNS team to offer support to the patient and family as needed.
Work closely with the EoL care facilitator and fast trackdischarge facilitator to ensure patients who are in the last weeks/days of lifehave a seamless handover of care, this may require the post holder to supportpatients on discharge from hospital
To be able to communicate in an empathic sensitive way tothe patient, family and significant others
To be aware of personal limitations and to seek assistancefrom other team members as needed
To ensure clear documentation of all interventions made withpatient, relative or staff member
