Qualifications

  • Minimum 10 years post‑qualification experience.
  • Minimum 5 years case management experience.
  • Knowledge/skills in support planning, risk assessment and management plans.

Responsibilities

  • Providing Case Management services to clients with complex disabilities.
  • Undertaking assessments of client’s needs and setting goals.
  • Ensure client’s well-being and safety is protected.

Skills

Experience working with brain injury
Strong organisational skills
Effective verbal and written communication
Problem-solving skills

Education

Relevant qualification in nursing, social work, occupational therapy, speech & language therapy or physiotherapy
Current registration with the appropriate professional body (e.g. SWE, HCPC, NMC)
BABICM Registered (Advanced BABICM is preferable)
