Job description
Role Overview

Eden Brown Synergy are currently recruiting for Barnet Council for the position of Qualified Social Worker (Hospital Discharge). The role is part of the Hospital Discharge Team, working closely with NHS colleagues and other health professionals to ensure safe, timely discharges from hospital to home or other care settings.

Rate: £33.51 per hour
Driving licence: Not required
Weekend work available

In this fast‑paced and collaborative environment you will carry out mental capacity assessments, provide advice and signposting, arrange home or residential care where needed, and hand over ongoing care to the Adult Early Intervention Team. This position is short‑term but high‑impact, offering valuable multidisciplinary hospital experience.

Key Responsibilities
  • Provide a high standard of social work in line with the Care Act 2014 and Barnet policies
  • Undertake professional social work assessments focusing on Adult Social Care Outcomes
  • Safeguard adults and carers at risk of harm, promoting Making Safeguarding Personal
  • Complete and review individualised, outcomes‑led care and support plans
