Summary
This role supports the Lead Pharmacist for Virtual Wards in delivering a specialised pharmacy service across all Virtual Hospital (VH) pathways. Additionally, it involves collaborating with the Lead Pharmacist for Community Health Services – Hertfordshire to enhance community services and integrate VH pathways. The primary objective is to provide a high-quality Medicines Optimisation Service aimed at improving medication-related outcomes for high-risk and prioritized patients, ensuring safe, cost-effective, and evidence-based use of medicines.
Key Responsibilities
Medicines Optimisation Service
- Deliver a high-quality Medicines Optimisation Service to enhance medication-related outcomes for high-risk patients.
- Ensure the safe, cost-effective, and evidence-based use of medications.
- Conduct pharmaceutical care for high-risk patients across all pathways, including Medicines Reconciliations, medication reviews, clinical screenings and interventions, counselling, providing clinical advice, therapeutic drug monitoring, and antimicrobial stewardship.
Support for Virtual Hospital Pathways
- Assist the Lead Pharmacist for Virtual Wards in providing specialized pharmacy services for all VH pathways.
- Participate in the development and implementation of medicines management policies and procedures for VH.
- Contribute to medicines optimisation projects within the VH framework.
Multidisciplinary Team Collaboration
- Actively participate in the multidisciplinary team and attend daily MDT meetings to provide pharmacological input.
- Support the wider virtual care initiatives, which may include the development and implementation of medicines management policies and procedures.
Cross-site Operational Support
- Work across sites between Watford General Hospital (WHTH) and Central London Community Healthcare (CLCH) based on operational needs.
