Job summary
A healthcare provider is seeking a skilled professional to manage community or hospital admissions and evaluate nursing interventions. Responsibilities
- Manage community or hospital admissions and liaise with agencies and service users.
- Evaluate nursing interventions for care plan effectiveness.
- Coordinate care with the MDT and hospital staff.
- Ensure interventions are within clinical practice scope.
- Provide guidance for individuals lacking capacity in health decisions.
- Develop risk assessments and management plans.
- Facilitate patient assessments before admission.
- Promote health awareness and action plans for service users.
- Support acute hospital services for complex care needs.
- Facilitate understanding of health needs for those with learning disabilities.
- Promote active participation of service users in their healthcare.
- Identify barriers for those with learning disabilities in accessing care.
- Collaborate with professionals to meet patients' needs.
- Lead hospital staff in preparing individuals with learning disabilities for treatment.
- Advocate for service users and escalate concerns appropriately.
- Promote rights and needs of individuals with learning disabilities regarding treatment.
Key Responsibilities
- To be responsible for community or hospital admissions, including liaison with other agencies, service users and carers.
- To evaluate the outcome of nursing interventions to ensure that care plan objectives have been met and that health gains have been achieved.
- To co-ordinate care with the MDT and hospital staff at points of attending appointments, admission and discharge.
- Any interventions must be within the post holder's scope of clinical practice ensuring that training is maintained and skills are in line with current guidance.
- To provide advice, practical support and guidance with regards to people who may lack capacity to make decisions about their health.
- To develop risk assessments if needed and appropriate risk management plans.
- To undertake or facilitate assessments of patients before admission.
- To promote awareness of health issues and the development of health action plans with service users and their carers.
- To support the acute hospital services in the assessment, planning, implementation and evaluation of care plans for service users with highly complex needs.
- To facilitate and promote people with a learning disabilities understanding of their own health needs.
- Promote the active participation of service users, their families and carers in the health care process.
- Contribute in identifying barriers for people with a learning disability accessing health care provisions and promote initiatives to overcome these barriers.
- To have an understanding of other professional roles, both in the community and within the acute trust, in order to work collaboratively meeting the needs of the person with a learning disability.
- Provide leadership to hospital staff in preparing people with a learning disability for hospital treatment e.g. consultation, explanations of treatment, using different forms of communication.
- Advocate on behalf of service users and carers to ensure that clients' care needs are met and that all concerns are escalated to an appropriate level.
- Promote the needs and rights of people with learning disabilities including the right to and consent to treatment relating to their mental capacity.
