Job description

We are looking for full time Technical Instructor to join us our Discharge Home to Assess Team, covering the East Hertfordshire corridor (from Cuffley to Bishops Stortford).

Working within a multi‑disciplinary team of health and social care professionals, providing support for clinical care to patients closer to home as part of the patient pathway from the acute sector through to discharge.

  • To provide support, assessment and treatment for patients within their own homes.
  • To maintain patient treatment records using System one.
  • To undertake a clinical caseload using knowledge and skills from theoretical and practical experience.
  • To participate in clinical supervision, and service–level education to ensure that clinical practice is maintained to a high standard.
Main duties of the job

Operational:

  • At all times to be aware of the needs of patients and their carers and help promote and encourage patient independence and where possible restore functional ability.
  • Discharge patients under the supervision of qualified staff.
  • Update patient records in a concise, accurate and timely manner in line with departmental standards and input appropriate statistics accordingly.
  • To deliver clinician‑guided activities and functional tasks in a patient's own environment.
  • To actively participate in and be aware of health promotion and health education initiatives within the service and apply these in practice.
  • To inform patient's carers and relatives in the safe use of mobility aids following previous instruction by a clinician.
  • To carry own caseload of non‑complex patients and prioritise this caseload, organising the day accordingly.

Management and Leadership:

  • To assist with the planning and implementation of induction programmes for new staff. To support the development and training needs of the team. To support the qualified therapists in assessing patients mobility in their normal residence.
  • To be responsible for equipment used in carrying out duties and to adhere to departmental equipment policy. To take an active role in supervision and the appraisal scheme as an appraisee and be responsible for complying with agreed personal development programmes to meet standard of knowledge and competencies.

Job responsibilities
