Job description

The purpose of this post is to provide community based specialist support to service receivers open to Community Mental Health Services in the Neighbourhood. The post holder will be actively involved in the delivery of care within the framework of Core Care Standards and in ensuring that the person receiving care is provided with an optimistic and hopeful environment that builds on their personal resources and promotes the management of their condition. The post holder will work under the supervision of registered professionals within the Neighbourhood Team

The primary aim of the post is to work within the Multi-disciplinary Team in order to create opportunities for service receivers to engage in plans of care that enable their recovery, and improve their quality of life and wellbeing. Key to the post is a commitment towards and understanding of Equality of experience, opportunity and outcomes for Service receivers. Those receiving service will have complex mental health needs and may also be in vulnerable situations and/or have physical health problems, which may also impact upon their social situation. The post holder will be responsible for reporting observations on progress to registered professional staff together with raising incidents or areas for concern.

Main duties of the job

To support the clinical team in delivering treatment and the plan of care by contributing to assessment, monitoring, engagement, relapse prevention, risk management and responding to clinical need with flexibility, and to report observations

To promote the health and wellbeing of service receivers and to contribute to maintaining a safe, caring and hopeful therapeutic environment

To work under the supervision of a registered professional in order to initiate and sustain therapeutic relations in a wide range of settings to enhance social engagement.

To support Registered professional with engaging service receivers in aspects of their assessments and care plans.

To co-facilitating psycho-educational, and recovery groups within the area of treatment as designated by the multidisciplinary team.

To support service receivers in developing skills as "peer supporters" or volunteers in order for them to contribute to service delivery.

Job responsibilities

To work with registered professional within the remit of the individuals care plan

To document relevant information relating to own activity in accordance with Trust policies and procedures, contributing to updating patient records, under supervision of the registered professional, using the appropriate manual or electronic system.

To participate as a member of the Multi-Disciplinary team.

To promote positive relationships with service users and carers

Foster and promote relationships with community support mechanisms, Voluntary agencies to enable social inclusion

To ensure that all care is provided in line with Trust values.

To ensure that all interactions with service receivers are used as opportunities to interact effectively to provide a high quality caring experience, and where possible, opportunities for positive change.

To promote effective communication which may include the need to encourage the service receiver and involve the use of tact. It may be necessary to adapt communication techniques in order to facilitate a service user understanding and engagement.
