Job description

Mediplacements are seeking an experienced Assessment & Safeguarding Social Worker to join a fast‑paced frontline team in London.

The successful candidate will undertake complex statutory assessments, lead safeguarding investigations, and manage high‑risk cases in line with the Care Act, Mental Capacity Act, and local authority safeguarding procedures. This role requires strong analytical skills, excellent decision‑making, and the ability to deliver high‑quality interventions that promote the safety, wellbeing, and independence of vulnerable adults.

Key Responsibilities
  • Complete thorough Care Act and safeguarding assessments, ensuring all risk factors are identified, analysed, and appropriately managed.
  • Lead Section 42 safeguarding enquiries, coordinating multi‑agency responses and ensuring timely, proportionate action.
  • Undertake Mental Capacity Act assessments, Best Interest decisions, and contribute to complex case discussions.
  • Develop person‑centred support plans that balance risk, independence, and statutory obligations.
  • Prepare comprehensive reports for safeguarding meetings, legal planning panels, and case conferences.
  • Work collaboratively with health, housing, police, community partners, and voluntary services to achieve safe and sustainable outcomes.
  • Maintain accurate and timely case recording in line with statutory duties and organisational policies.
  • Provide professional judgement and leadership on high‑risk cases, escalating concerns where appropriate.
  • Ensure all interventions reflect best practice, current legislation, and the highest standards of social work ethics.
Ideal Candidate Profile

We are particularly interested in candidates with experience similar to the following:

  • A highly experienced Mental Health Social Worker with a proven track record in complex Care Act assessments, safeguarding investigations, and risk management.
  • A seasoned practitioner bringing 19 years of social work experience, including 10 years as a senior safeguarding lead, with advanced skills in complex assessments, Mental Capacity Act compliance, and Care Act best practice – able to deliver immediate impact.
Requirements
  • Qualified Social Worker with active SWE registration.
  • Significant experience in adult safeguarding, assessment work, and complex case management.
  • Strong working knowledge of the Care Act, Mental Capacity Act, and adult safeguarding frameworks.
  • Confident decision‑maker with excellent written, verbal, and analytical skills.
  • Ability to work in a fast‑paced environment and manage competing priorities effectively.
