Job description

Are you a confident and motivated healthcare professional who works well in a multi-disciplinary team environment?

Stevenage South PCN are seeking a skilled advanced practitioner to join our supportive team and deliver high-quality, safe, patient-centred care.

Based in Stevenage, Hertfordshire, this position is offered on a part-time basis for 16 - 22.5 hours/ 2-3 days per week (exact days to be agreed with the right applicant).

  • This is a contract position, fixed-term until 31st March 2026 but has the potential for extension.
Main duties of the job
  • Conduct thorough clinical assessments and formulate management plans for patients with minor illnesses.
  • Assess, diagnose, and manage patients presenting with a variety of minor illnesses across primary and urgent care settings.
  • Prescribe independently, providing efficient and effective treatment, reducing waiting times and improving patient outcomes.
  • Work collaboratively with GPs, nurses, pharmacists, and other team members to deliver seamless care.
  • Provide health promotion advice and support patient self-care.
  • Maintain accurate clinical records and comply with all governance and regulatory standards.

Experience
  • Experience and training in minor illness assessment and management.
  • Strong clinical reasoning skills and sound knowledge of current prescribing guidelines (e.g., NICE).
  • Excellent communication and interpersonal skills, with the ability to work well within a team and support fellow colleagues (clinical and non-clinical)
  • Commitment to ongoing professional development and high-quality patient care.
Qualifications
  • Registered healthcare professional (Nurse, Pharmacist, Paramedic, or other allied health professional) with a valid Independent Prescribing qualification.
