Job description

Advanced Pharmacist - Primary Care & Community

Help shape prescribing in primary care across Wrexham and Flintshire by joining our multidisciplinary team to deliver evidence-based projects. We seek a forward-thinking pharmacist to lead clinical projects over 6-12 months with measurable outcomes in BCUHB.

You will also need an interest in clinical delivery and broadening your scope of prescribing practice and enhancing any specialist areas. You'll enjoy a varied role across Wrexham and Flintshire regions, adapting to areas where your expertise makes the greatest impact. Beyond your own practice you will need to be able to influence and engage with other prescribing professionals working in the community and other settings to support changes in their prescribing practice.

We are looking for a confident communicator, passionate about delivering long-term change and able to plan ahead to implement step-wise changes in the healthcare system. You will contribute to training and development of trainees, other professionals and peers alike within our team and support the operational management staff and services.

You will benefit from linking in with a wide range of experienced generalist and specialist prescribers to help you develop your own practice and facilitate your advanced pharmacist credentialling.

The team also provides clinical services to 3 community hospitals and 6 health board managed practices, which is operated on a rota basis.

Main duties of the job

Project Leadership - You will be involved in identifying, proposing and implementing prescribing projects over an agreed time span. You will need to be able to present information and rationales for project proposals and deliver continued update reports. During a project you will operate as clinical escalation for pharmacy technicians and deliver clinical intervention to priority patients within your scope of practice.

Clinical Duties - You will deliver clinical reviews in different formats including face-face clinics, telephone reviews and medication reviews of clinical notes depending on the needs of the service and the agreed project. Your job plan will also include dedicated time for clinical development to maintain and expand on your scope of practice.

Following induction and dependent on service requirements you may also need to contribute to rota duties such as clinical pharmaceutical services at community hospitals and in GP surgeries.

Training Support and Leadership - You will provide mentorship, supervision or managerial oversight to trainees and other staff members. You will be expected to support education and training, be an active leader within assigned responsibilities and contribute to research within the organisation. Policy review and implementation may also be required.

The post is open to pharmacists from all sectors, with potential opportunities to work across different areas of pharmacy.

Qualifications and/or Knowledge

• MPharm degree or equivalent
• Registered with the General Pharmaceutical Council
• Post Graduate MSc/Diploma in Clinical Pharmacy or equivalent experience and/or qualifications
• Member of the Royal Pharmaceutical Society & registration with the Faculty
• Accredited or eligible to be accredited as a Higher Education Institute Clinical Pharmacy diploma/MSc tutor
• Non-medical prescribing qualification (or working towards)

Experience

• Must be able to demonstrate suitable level of experience working in the managed sector and/or community pharmacy
• Experience in relevant clinical area
