Manor Farm Medical Centre is looking to recruit an enthusiastic, compassionate and committed Advanced Nurse Practitioner on a permanent basis for 3 - 4 days, Monday to Friday. We are looking for someone who is able to support the wider clinical team, triage and prioritise the needs of our patients. Thepost holder will commit to demonstrating critical thinking in the safe clinicaldecision-making process. They will communicate and work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures.

Main duties of the job

You will need to be a degree qualified Advanced Nurse Practitioner with relevant clinical skills and expertise in order to work autonomously to help provide ongoing clinical care and manage acute demand. You will be confident carrying out telephone triage, clinical consultations, providing assessment, diagnosis and treatment arranging specialist referrals when indicated. Prescribing qualification isNOT essential, but if had, prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and withinscope of practice.

About us

Manor Farm Medical Centre is situated in a busy market town with approximately 7900 patients serviced by our main site and a branch surgery. We are a friendly dispensing practice with 3 GP partners, 2 salaried GPs a very busy team of ANPs, Practice Nurses, Nurse Associates and HCAs, Receptionists, Dispensers and Administrators. We are a forward thinking practice and endeavour to provide a comprehensive service, as well as being a teaching and research practice associated with the UEA and involved in the training of F2 doctors. We use EMIS Web and hold a 'Good' CQC rating.

Job Summary

Thepost holder is an experienced caring, compassionate and committed nurse who,acting within their professional boundaries, will provide care for thepresenting patient from initial history taking, clinical assessment, diagnosis,treatment and evaluation of care. They will demonstrate safe, competentclinical decision-making and expert care, including assessment and diagnosticskills, for patients within the general practice. The post holder will committo demonstrating critical thinking in the safe clinical decision-makingprocess. They will communicate and work collaboratively with the generalpractice team to meet the needs of patients, supporting the delivery of policyand procedures.

Key Responsibilities

ClinicalPractice

  • Assess, diagnose, plan,implement and evaluate treatment/interventions and care for patientspresenting with an undifferentiated diagnosis
  • Telephone Triage
  • Clinically examine and assesspatient needs from a physiological and psychological perspective, and planclinical care accordingly
  • Assess, diagnosis, plan,implement and evaluate interventions/treatments for patients with complexneeds. Proactively identify, diagnose and manage treatment plans forpatients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan

  • Work with patients in order tosupport compliance with and adherence to prescribed treatmentsProvide information and advice on prescribed or over-the-countermedication on medication regimens, side-effects and interactions
  • Prioritise health problems andintervene appropriately to assist the patient in complex, urgent oremergencysituations, including initiation of effective emergency care
  • Support patients to adopthealth promotion strategies that promote healthy lifestyles, and applyprinciples of self-care. Support and manage health needs of womenpresenting for family planning, cervical cytology or sexual healthconsultation
  • Assess, identify and referpatients presenting with mental health needs in accordance with the NSFfor Mental Health. Implement and participate in vaccination andimmunisation programmes for both adults and children
  • Advise, support and administervaccinations for patients travelling abroad, where appropriate
  • Promote and deliverevidence-based care for patients presenting with aural conditions
  • Meet the needs of patientspresenting for opportunistic wound care
  • To undertake Home Visits tohousebound patients as needed and Care Homes if required

  • Utilise and demonstratesensitive communication styles, to ensure patients are fully informed andconsent to treatment
  • Communicate with and supportpatients who are receiving bad news
  • Communicate effectively withpatients and carers, recognising the need for alternative methods ofcommunication to overcome different levels of understanding, culturalbackground and preferred ways of communicating
