A reputable healthcare provider in Chester seeks an experienced Advanced Nurse Practitioner to join their Care Home Team. The successful candidate will manage patients with long-term conditions and acute illnesses while having excellent clinical and interpersonal skills. Applications from both experienced and newly qualified ANPs are welcome. The role offers a competitive salary and six weeks of annual leave while contributing to holistic patient care in care homes.
Fountains Medical Practice has a growing practice populationand is based in Fountains Health. In addition to providing an excellent primarycare service to our patients, we provide a successful service to all the carehomes in Chester city through the Fountains Integrated Care Homes Service(FICHS). The FICHS team works with the multidisciplinary services in thecommunity, secondary care specialists and voluntary sector to provide holisticcare to patients in care homes. We work closely with other practices as part ofa successful and expanding PCN and we are a training practice, working with Medical Students from Chester University andthe University of Buckingham.
Due to continued expansion, an exciting opportunity hasarisen for an experienced Advanced Nurse Practitioner to join our Care Home Team.We would ideally expect the successful candidate to have experience of managingpatients with long term conditions, acute illnesses, a working knowledge of QOFand the current Direct Enhanced Services. Experience or interest in care of theelderly and palliative care is desirable. Applications are welcome from bothnewly qualified and experienced ANP's.
The candidate must have excellent clinical and interpersonalskills to ensure safe, effective and high standards. They should be willing towork hard to develop the role to compliment the clinical and administrativesupport staff.
