A healthcare practice in the UK is seeking an Advanced Nurse Practitioner/Advanced Paramedic Practitioner for their triage team. You will be involved in assessing and treating minor conditions, providing support and advice to patients, and delivering a high standard of patient care. Bramblehaies Surgery is a semi-rural practicein the heart of Devon serving a population of 7,500 patients, providing highstandards of care (CQC rating of Good) from a partner owned modern,purpose-built building.
Our team currently consists of four GP partners,two salaried GPs, Nurse Practitioner, Practice Manager, Patient ServicesManager, Data Quality Administrator, Clinical Pharmacist and PharmacyTechnician, a team of three Practice Nurses headed up by a Lead Nurse, threeHCAs, a General Practice Assistant, a committed admin/reception team and ourmuch-needed housekeeping team.
We are a member practice of Culm ValleyPrimary Care Network and supported with Social Prescribers, Health andWellbeing Coach, First Contact Physio, Mental Health Worker, and a Care HomeTeam consisting of a Nurse Practitioner and an Occupational Therapist.
We are a training practice and aVisa-sponsoring practice hosting GP Registrars and medical students.

The Advanced Nurse Practitioner/Paramedic Practitioner will:
a.Workas part of a multi-disciplinary team (MDT) within Bramblehaies Surgery and aspart of the wider PCN clinical team.
b.Assessand triage patients including same day triage and, as appropriate, providedefinitive treatment (including prescribing medications, following policy,patient group directives, national e.g. NICE and local clinical guidelines andlocal care pathways) or make necessary referrals to other members of theprimary care team.
c.Manageundifferentiated undiagnosed conditions and identify red flags and underlyingserious pathology and take appropriate action.
d.Usecomplex decision making to inform the diagnosis, investigation, completemanagement of episodes of care within a broad scope of practice.
e.Activelytake a personalised care approach and population-centred care approach toenable shared decision making with the presenting person.
f.Beable to perform specialist health checks and reviews within their scope ofpractice and in line with local and national guidance;
g.Supportthe delivery of anticipatory care plans and provide an alternative model tourgent and same day GP home visit for the Practice/PCN.
h.Completethe relevant training to provide multi-professional clinical practice and CPDsupervision to other roles within primary care, for example Practice Nurses andthe personalised care roles.
i.Advisepatients on general healthcare and promote self-management where appropriate,including signposting patients to the PCNs social prescribing service and,where appropriate, other community or voluntary services.
j.Communicateat all levels across organisations ensuring that an effective, person-centredservice is delivered.
k.Communicateproactively and effectively with all colleagues across the multi-disciplinaryteam, attending and contributing to meetings as required.
l.Maintainaccurate and contemporaneous health records appropriate to the consultation,ensuring accurate completion of all necessary documentation associated withpatient health care and registration with the practice.
m.Communicate effectively with patients, and whereappropriate family members and their carers, where applicable, complex andsensitive information regarding their physical health needs, results, findings,and treatment choices.
