Adult Social Worker - 0-25 Transforming Care Team

Location: Hertfordshire (Countywide)

Pay: £32 per hour

Role: Adult Social Worker - 0-25 Transforming Care Team

About the Role

We are seeking an experienced Adult Social Worker to join the 0-25 Together Service within the Transforming Care Team. This role focuses on supporting young adults aged 18-25 with complex mental health needs who are at risk of hospital admission.

You will work both remotely and from office locations across the county, so a full driving licence and access to a car are essential. Due to the nature of the work, we require a female social worker to support individuals living with trauma who may be triggered by male workers.

Key Responsibilities
  • Complete Care Act and Capacity assessments.
  • Undertake Community DoLs applications and prepare reports for court.
  • Support transitions in and out of hospital admission.
  • Carry out risk assessments and work within a multi‑agency team including mental health services, learning disability teams, schools, colleges, and providers.
  • Apply knowledge of CETR, DSR, and the Mental Health Act.
What We're Looking For
  • Qualified Adult Social Worker with current registration.
  • Experience in adult safeguarding and working with learning disabilities and transition cases.
  • Strong understanding of complex mental health needs and risk management.
  • Flexible and able to work under pressure.
