Key Responsibilities

• Monitor Patient Data: Review and analyze real-time data collected through remote monitoring devices, including vital signs, glucose levels, blood pressure, weight, and other relevant health metrics.
• Patient Support & Education: Provide timely responses to patient queries related to their health status, offering guidance on device usage and health management.
• Alert Management: Assess alerts and notifications for potential issues, such as abnormal readings or deviations from prescribed care plans, and elevate critical situations to healthcare providers.
• Collaboration with Healthcare Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and support for patients. Provide regular updates on patient progress and concerns.
• Data Entry & Documentation: Maintain accurate and comprehensive patient records in the electronic health record (EHR) system, ensuring compliance with healthcare regulations and privacy standards (HIPAA).
• Care Plan Adherence: Ensure that patients are following prescribed care plans by providing remote check‑ins, reminders, and motivational support.
• Technology Management: Troubleshoot technical issues related to remote monitoring devices and software, providing first‑line support to patients and escalating issues when necessary.
• Quality Improvement: Contribute to ongoing quality improvement initiatives to enhance patient outcomes and optimize the remote monitoring process.
• Patient Education & Empowerment: Empower patients to take an active role in managing their health through education on lifestyle changes, medication adherence, and symptom management.

Qualifications

• Education: A degree or certification in Nursing, Healthcare Administration, or a related field (e.g., Medical Assistant, Health Information Management) preferred.
• Experience:
  • At least 1–2 years of experience in a healthcare setting, preferably in patient monitoring, telehealth, or chronic disease management.
  • Familiarity with remote patient monitoring tools and telemedicine platforms is a plus.
• Skills & Competencies:
  • Strong knowledge of medical terminology, chronic disease management, and remote monitoring devices.
  • Excellent communication skills, both verbal and written.
  • Detail‑oriented with the ability to analyze data and identify potential health concerns.
