Jobtailor is seeking an experienced Registered Nurse to perform clinical triage for Medical Group patients. You will assess symptoms, provide medical advice, coordinate care with the care team, and review clinical test results for provider review.

This remote role requires an active RN license, 2+ years of RN experience, strong communication skills, and proficiency with Aprima EHR. You will educate patients, handle inquiries, and contribute to quality improvement initiatives.

Qualifikationen

• Graduate of approved nursing education program and RN licensure.
• Current valid RN license; able to obtain in all service states within 6 months.
• 2+ years of RN experience; nurse triage experience.
• Experience with Aprima EHR; proficiency in EHR systems.
• Knowledge of Medicare/Medicaid insurances and programs.
• Strong communication, multitasking, crisis management, and problem solving.
• Ability to work remotely and independently.
• Familiarity with primary healthcare principles.

Aufgaben

• Triage inbound sick calls from Medical Group patients.
• Assess patient symptoms and provide medical advice.
• Coordinate care with the care team and determine level of care.
• Review and summarize clinical test results for provider review.
• Prioritize inbound calls and respond to voicemails and test results.
• Conduct telephone assessments using probing questions and the EMR.
• Apply evidence-based guidelines to determine triage priority.
• Identify crisis situations and connect patients with emergency services.
• Educate patients and caregivers on health maintenance, disease prevention, self-care, medications, and follow-up steps.
• Make outbound calls to communicate provider orders.
• Collaborate with care teams to complete follow-ups and promote quality outcomes.
• Analyze results data and communicate findings to providers.
• Enter patient results and quality metrics.
• Address and elevate patient complaints, grievances, and concerns.
• Participate in projects and quality improvement initiatives.
• Perform additional duties assigned by the Clinical Triage Manager and/or Director.

Requirements

• Graduate of approved or accredited nursing education program as a Registered Nurse
• Current valid RN license
• Must be able to obtain and maintain RN license within all service states within 6 months from hire
• Satisfactory completion of required nursing continued education requirements
• Ability to react decisively and quickly in urgent and emergent situations
• Strong comprehension of medical terminology and pharmaceuticals
• Demonstrated experience with multi-tasking
• Strong communication skills (written & spoken)
• Demonstrated proficiency in using electronic health records (HER) system
• Ability to communicate effectively with providers, patient care teams, patients, patients’ family members and facility staff
• Ability to work remotely and independently
• Knowledge of basic principles, practices, and techniques in primary healthcare
