Jobtailor is seeking a registered nurse to provide comprehensive population health management and care coordination for a targeted high-risk population. You will conduct health risk assessments, educate patients, and navigate benefits to improve health outcomes.

You will collaborate with client leadership, coordinate with primary care and specialists, and lead wellness initiatives while ensuring compliant documentation and performance metrics are tracked.

• Provide nursing care and care coordination to a targeted population identified through risk stratification reports
• Conduct comprehensive patient health risk assessments and provide health education and benefits navigation
• Present population health findings, trends, and recommendations to client leadership and matrix partners
• Participate in client meetings to review outcomes, intervention performance, and emerging health priorities
• Partner with client stakeholders to prioritize population health initiatives and engagement strategies
• Perform biometric health risk assessments and education
• Follow established nursing protocols and procedures
• Monitor medication compliance and provide patient education
• Educate patients on chronic conditions, abnormal biometric values, health needs, and behavioral lifestyle modifications
• Use motivational interviewing to promote sustainable changes in diet, exercise, and stress management
• Identify health education needs through targeted health assessments
• Identify and help overcome psychosocial barriers to care
• Establish health improvement plans and personalized evidence-based health goals with patients
• Track and monitor screening compliance
• Facilitate preventive health screenings, including cancer screenings
• Lead and support wellness activities, group coaching, wellness challenges, seminars, and client-site campaigns
• Coordinate communication with external providers, including primary care and specialists
• Coordinate with community providers and resources to address patient needs and care gaps
• Reinforce provider care plans and ensure adherence
• Follow up with patients regarding after-hours care recommendations
• Refer patients to clinical programs and services
• Help patients navigate benefits and access resources
• Advocate for timely and appropriate patient care
• Support health-related site events, including open enrollment, wellness committees, flu shot events, and health fairs
• Use reports and dashboards to identify and proactively engage highest-risk patients
• Prioritize outreach and interventions based on risk stratification
• Ensure regulatory compliance and appropriate electronic documentation

Requirements

• Current unrestricted compact state RN licensure required
• Three or more years of clinical practice experience
• Bachelor's degree in a health-related field
• Current BLS/CPR/AED Certification
• Excellent computer skills, including Word and Excel
• Excellent interpersonal skills
• Ability to work in a team environment
• Flexibility and enthusiasm for learning new information and developing new skills quickly
• High energy level and passion for health improvement
• Positive role model in demonstrating healthy behaviors
• Self-directed and able to succeed in an independent role
• Population health management, care management, clinical program management and/or health analytics experience preferred
• Care management or case management certification preferred
• Experience with healthcare analytics and reporting tools preferred
• Knowledge of employer-sponsored health plans and wellness strategies preferred
• Experience with Epic, population health platforms, or care coordination systems preferred
• Cable broadband or fiber optic internet service with minimum speeds of 10Mbps download/5Mbps upload for home work
