Job Summary:

Utilizing clinical knowledge and skills, Social Workers develop collaborative relationships with aging patients and their families to promote patient-centered goals. In this role, the Social Worker will facilitate psychosocial assessments of the emotional, physical, cognitive, psychosocial and behavioral needs of patients and their families. As an integrated member of the interprofessional team, the Social Worker will provide discharge planning, referrals and resource navigation, advanced care planning and counseling to patients and their families, with a focus on ensuring successful transitions in care for patients with responsive behaviors, Dementia and mental health needs. The Social Worker will facilitate referrals to Ontario Health at Home, along with community resource navigation as required, working closely with the Virtual Behavioral Medicine (VBM) and Behavior Support Ontario (BSO) teams. The Social Worker in this position demonstrates a high degree of practice excellence has experience working with responsive behaviors and Dementia and functions as a clinical resource for the interprofessional team as well as the patient and family. This position reports directly to the Manager and Interprofessional Practice Chief for Social Work, and involves cross-coverage across the inpatient Hospital units.

Responsibilities
  • Provides pre-admission psychosocial assessments as well as ongoing crisis intervention, discharge planning and counseling to patients and their families, including advanced care planning and end-of-life care, primarily with patients with responsive behaviours and mental health needs
  • Provides support/consultation to other disciplines on the team re: psychosocial issues, particularly related to complex patient/family situations and responsive behaviours
  • Acts as the team expert on discharge planning and patient flow, including reviewing referrals and liaising with VBM
  • Acts as a linkage and communication conduit between the family and the care team
  • Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services, including VBM and BSO
  • Participates in teaching and other student supervision activities for health professional students assigned to this unit
Qualifications
  • Masters of Social Work degree
  • Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’)
  • Experience in Mental Health preferred
  • Experience with Responsive Behaviours preferred
  • Experience in discharge planning and system navigation preferred, including knowledge of VBM and BSO
  • Experience working in a healthcare setting or mental healthcare setting preferred
  • Excellent interpersonal, communication and problem-solving skills
  • Basic computer skills – Meditech, Microsoft Word and Excel, RM&R preferred
