Job summary

TELUS Health is seeking a Disability Case Manager to support employees returning to work. You will assess barriers to return, coordinate interventions, and partner with HR and clinicians to develop effective return-to-work plans.

You will manage a diverse caseload, communicate with HR, and assist with transitions to long-term disability, while contributing to a collaborative, learning-focused team environment. The role offers hybrid work options across Canada.

Qualifications

• 2-5 years experience in Disability Case Management or related field
• Education in occupational health, rehabilitation, disability management, or HR related areas is advantageous
• Registration in disability management or health sector is a plus
• Strong communication and customer service focus are essential
• Interpersonal skills, empathy, and sensitivity are required
• Detail oriented and able to work independently in a fast-paced environment
• Excellent problem solving and critical thinking for complex cases

Responsibilities

• Conduct case assessments by contacting employees and managers to explain the disability management process and gather information
• Develop understanding of the situation and factors affecting return to work
• Provide interventions such as further assessment, medical management, and coordination of occupational/psychological interventions
• Determine RTW goals with employee and employer and support RTW planning
• Communicate with HR regarding case management issues impacting workplace/RTW
• Manage transition to long-term disability and provide information to insurer
• Maintain accountability for assigned caseload and stakeholder relationships
• Attend Case Management and Disability Management department meetings
• Perform other projects and tasks as assigned
