Job summary

VON Canada is seeking a Registered Practical Nurse to provide comprehensive home and community care across London Middlesex and Elgin. You will deliver holistic, client-centered care directly in clients' homes and in clinics, with opportunities for virtual care and outreach activities.

You will assess needs, administer medications, support rehab and chronic disease management, and collaborate with other health professionals to ensure safe, high-quality care for children, adults and seniors.

Qualifications

• Must be licensed as a Registered Practical Nurse in Ontario
• Valid driver’s license and access to a reliable vehicle
• Successful Criminal Records Check or equivalent
• Ability to wear PPE and follow infection control protocols

Responsibilities

• Perform client assessments in home and community settings
• Provide primary care in collaboration with other healthcare professionals
• Administer medications and monitor effects
• Deliver wound care and chronic disease management
• Collect specimens when needed and support rehabilitation programs

Our Visiting nurses provide client care in the client’s own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families and much more.

What we're looking for

• Must be licensed as a Registered Practical Nurse in the province of practice
• Community health experience is an asset
• Must enjoy driving, have a valid driver's license and access to a reliable vehicle
• A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
• Must be able to wear Personal Protective Equipment (PPE)

Working conditions and physical demands

This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements.
