Job summary

VON Canada is seeking a Part-time Visiting Nurse (0.4 FTE) to deliver holistic, person-centred home and community care. You'll assess clients, coordinate with healthcare teams, and support families across homes, clinics, and virtual visits.

As a licensed RN with a valid driver's license and access to a vehicle, you’ll work in flexible schedules including days and weekends, with PPE and safety measures in place, and comprehensive benefits provided.

Qualifications

• Must be licensed as a Registered Nurse in the province of practice.
• Current driver's license and access to a personal vehicle.
• Willingness to wear PPE and adhere to safety protocols.
• Criminal Records Check is required; Vulnerable Sector Search may be necessary.

Responsibilities

• Client assessments to develop individual care plans.
• Provide primary nurse care in clients' homes, clinics, or virtually.
• Wound care, IV therapy, dialysis management, and palliative support.
• Chronic disease management and education for patients and families.
• Documentation and coordination with other healthcare professionals.

Our Visiting nurses provide client care in the client’s own home. They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counseling to individuals and families, and much more.

What you'll do

• Client assessments to develop individual care plans

• Simple and advanced wound care, including complex and negative pressure wounds
• Chronic Disease Management, e.g. diabetes care and education
• IV therapy and CVAD management
• Phlebotomy and specimen collection
• Dialysis Management
• Rehabilitative programs
• Palliative and End of Life Care
• Medication management
• And many other critical nursing skills

What we're looking for

• Must be licensed as a Registered Nurse in the province of practice
• Community health experience is an asset
• Must enjoy driving, have a personal vehicle and possess both a current driver's license and proof of vehicle insurance
• A satisfactory Criminal Records Check is required; a Vulnerable Sector Search and/or Child Abuse Registry Check may also be necessary
• Must be able to wear Personal Protective Equipment (PPE)
