Job description

We are seeking an experienced Practice Nurse tojoin our dedicated team. This is an exciting opportunity for a motivated andskilled nurse to contribute to the delivery of safe, effective and high-qualitycare within a primary care setting. The successful candidate will work withinthe NMC Code of Conduct, supporting service development and promoting integratedcare pathways for patients.

Please note this is a part time role offering 20 hours a week and the salary advertised is the pro rata'd amount.

Main duties of the job

The Practice Nurse will:

  • Deliver primary health care interventions and treatments in line with clinical protocols.
  • Provide health education and promotion to patients and carers.
  • Undertake clinical duties such as wound care, immunisations, cervical screening, phlebotomy, ECGs, and chronic disease monitoring (e.g., diabetes, asthma, COPD).
  • Work collaboratively with GPs, nurses, and other healthcare professionals to ensure seamless patient care.
  • Maintain accurate patient records and contribute to audits and quality improvement initiatives.
  • Support infection prevention and control, safeguarding, and health & safety standards.
  • Participate in team meetings, training, and professional development activities.

Job responsibilities
  • Responsible for providing and delivering primary health care interventions and care effectively and efficiently
  • Working collaboratively with the multi-disciplinary general practice team and other healthcare professionals to promote integrated pathways of care to meet patient needs.
  • Effective communication with patients, carers and all members of the MDT
  • Effective prioritisation and completion of daily work in line with clinical / patient need and practice priorities
