Occupational Therapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark

Location: Sudbury, ON

Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Occupational Therapists to complete Independent Living Services (ILS) assessments throughout the province of Ontario.

The ILS assessments are to be completed in the residential homes of injured workers. This is a casual part-time opportunity based on referrals.

OT Responsibilities:
  • Complete a variety of in-depth and thorough assessments such as Personal Care Needs Assessments, Home Safety Assessments, Home Accessibility Assessments, and Mobility Assessments
  • Develop comprehensive reports which include assessment findings, recommendations and service plan in accordance with CAR, the referral source and COTO standards
  • Collaborate with interprofessional team members, community resources and the referral source as needed.

All Occupational Therapists that are in good standing with the College of Occupational Therapists are eligible to apply. In order to perform mobility assessments you will need to be an Assistive Devices (ADP) Authorizer, however this is not required to apply.
