Nurse Practitioner – Full‑Time Remote; Ontario (currently licensed in ON)

Job Description

The Nurse Practitioner is licensed to work in Canada and provides health care advice, education and support to patients across Canada in jurisdictions that allow for cross province care. As point of contact for patients, you will provide convenient and timely supportive services that range from minor acute care treatments to health coaching for various disease states. You will be responsible for triaging incoming requests from patients, assessing their needs and providing the appropriate clinical recommendations and coaching to support resolution of symptoms and/or improvement in perceived health. Patients will be able to contact a Registered Nurse via a variety of channels including phone, email, text and videoconference. Where necessary, you will be referring patients to healthcare facilities and community‑based resources. Working collaboratively with other health care professionals (physicians, pharmacists and dietitians), you will provide a meaningful patient experience, while using your critical thinking skills to develop, implement, and evaluate comprehensive care plans.

What you will do:

• Possess strong clinical expertise and experience in a range of health conditions
• Possess compassion and empathy in order to treat our patients and promote healthy lifestyles.
• Utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate.
• Engage in service/program development of the clinical telemedicine program, including liaising with healthcare providers and patients, and being a champion and advocate for the program.
• Ensure compliance with provincial and federal privacy legislation including the Mental Health Act.

What you will need:

• Current unrestricted professional nursing license to practice in the province of work with willingness to obtain and maintain additional provincial NP licenses as required
• Availability that includes weekday days and occasional evenings that support our regular business hours of 8am – 8 pm EST; occasional weekends, as well as on‑call shifts that may be required
• 2 years direct clinical experience as Nurse Practitioner preferred
• Ability to apply knowledge of developmental stages, behavioral sciences, pathophysiology, psychopathy, epidemiology, and evidence‑based practice to clinical diagnosis and patient management
• Ability to apply knowledge of pharmacology, pharmacokinetics and pharmacodynamics to prescribing and evaluating treatment responses
• Demonstrated leadership experience preferred
• Ability to provide an organized, efficient approach to telemedicine
• Possession of excellent skills in assessment, communication, critical thinking and decision making
• Ability to demonstrate understanding of SDM Specialty Health Network policies, legal considerations and professional ethics
• Excellent communication skills in English (bilingual French/English preferred)
• Proficiency of computer skills including Microsoft 365; experience with web‑based applications (i.e. Sales Force) an asset
• Ability to remain objective and be a team player

Description du poste

L'infirmière praticienne est autorisée à exercer au Canada et fournit des conseils, des informations et un soutien en matière de soins de santé à des patients partout au Canada, dans les provinces qui autorisent les soins interprovinciaux. Si nécessaire, vous orienterez les patients vers des établissements de santé et des ressources communautaires. En collaboration avec d'autres professionnels de la santé (médecins, pharmaciens et diététiciens), vous offrirez une expérience significative aux patients, tout en utilisant vos capacités de réflexion critique pour élaborer, mettre en œuvre et évaluer des plans de soins complets.

Ce dont vous aurez besoin :

• Permis d'exercice professionnel sans restriction en cours de validité dans la province où vous travaillez, et volonté d'obtenir et de conserver les permis provinciaux supplémentaires requis pour exercer en tant qu'infirmier praticien
• Disponibilité incluant les jours de semaine et parfois les soirées, conformément à nos heures d'ouverture habituelles de 8 h à 20 h (HNE); disponibilité occasionnelle le week‑end, ainsi que pour des gardes qui pourraient être nécessaires
• Deux ans d'expérience clinique directe en tant qu'infirmier praticien, de préférence
• Capacité à appliquer ses connaissances des stades de développement, des sciences comportementales, de la physiopathologie, de la psychopathie, de l'épidémiologie et de la pratique fondée sur des preuves au diagnostic clinique et à la prise en charge des patients
• Capacité à appliquer ses connaissances en pharmacologie, pharmacocinétique et pharmacodynamique à la prescription et à l'évaluation des réponses aux traitements
• Expérience avérée en matière de leadership préférée
• Capacité à adopter une approche organisée et efficace de la télémédecine
• Excellentes compétences en matière d'évaluation, de communication, de réflexion critique et de prise de décision
• Capacité à démontrer une compréhension des politiques, des considérations juridiques et de l'éthique professionnelle du SDM Specialty Health Network
