Nurse Practitioner – Full‑Time Remote; Ontario (currently licensed in ON)
The Nurse Practitioner is licensed to work in Canada and provides health care advice, education and support to patients across Canada in jurisdictions that allow for cross province care. As point of contact for patients, you will provide convenient and timely supportive services that range from minor acute care treatments to health coaching for various disease states. You will be responsible for triaging incoming requests from patients, assessing their needs and providing the appropriate clinical recommendations and coaching to support resolution of symptoms and/or improvement in perceived health. Patients will be able to contact a Registered Nurse via a variety of channels including phone, email, text and videoconference. Where necessary, you will be referring patients to healthcare facilities and community‑based resources. Working collaboratively with other health care professionals (physicians, pharmacists and dietitians), you will provide a meaningful patient experience, while using your critical thinking skills to develop, implement, and evaluate comprehensive care plans.

L'infirmière praticienne est autorisée à exercer au Canada et fournit des conseils, des informations et un soutien en matière de soins de santé à des patients partout au Canada, dans les provinces qui autorisent les soins interprovinciaux. En tant que personne‑ressource pour les patients, vous fournirez des services de soutien pratiques et rapides, allant des traitements mineurs de soins aigus au coaching santé pour diverses pathologies. Vous serez chargé(e) de trier les demandes des patients, d'évaluer leurs besoins et de fournir les recommandations cliniques et le coaching appropriés pour aider à soulager les symptômes et/ou améliorer la santé perçue. Les patients pourront contacter une infirmière autorisée par divers moyens, notamment par téléphone, par courriel, par SMS et par vidéoconférence. Si nécessaire, vous orienterez les patients vers des établissements de santé et des ressources communautaires. En collaboration avec d'autres professionnels de la santé (médecins, pharmaciens et diététiciens), vous offrirez une expérience significative aux patients, tout en utilisant vos capacités de réflexion critique pour élaborer, mettre en œuvre et évaluer des plans de soins complets.
