Job summary

A community health organization in Ontario is seeking a Mobile Wellness Intake/Care Coordinator to serve as the primary contact for Wellness services. This role includes conducting assessments, coordinating care, and engaging with community partners. The ideal candidate will have a Bachelor of Social Work and experience in mental health or addictions. Knowledge of First Nation communities is preferred.

Qualifications

  • Minimum of three years of experience providing community-based services in mental health or addictions.
  • Registration in good standing with the Ontario College of Social Workers.
  • Ability to work collaboratively as part of a multidisciplinary team.

Responsibilities

  • Conduct comprehensive intakes and assessments for individuals seeking Mobile Wellness services.
  • Coordinate access to traditional healing and culturally relevant programming.
  • Participate in community engagement activities to promote Mobile Wellness services.
