Job summary

TASC in Belleville, Ontario is seeking a Full-Time Case Manager for the Housing Division. You will provide specialized case management and care coordination to secure stable housing and access essential services, using a trauma-informed, person-centered approach.

Ideal candidates hold a high school diploma or GED, a valid driver's license, and may be required to obtain CADC certification. You must pass an IDOC background check, be proficient with Microsoft Office, and have experience

Qualifications

  • Bachelor? No, High school diploma or GED required.
  • CADC certification within two years of employment with TASC may be required.
  • Valid driver2s license is required.
  • Pass an IDOC Background check is mandatory.
  • Proficiency with Microsoft Office applications is expected.
  • Experience facilitating groups is preferred.
  • Knowledge of ASAM criteria, DSM-V related to substance use, or willingness to learn is needed.
  • Comfort working inside a correctional setting and with persons convicted of serious offenses.
  • Strong verbal, written and interpersonal communication skills.
  • Good time management and ability to meet deadlines.

Responsibilities

  • Provide specialized case management and care coordination for housing stability.
  • Conduct housing screenings and assessments to inform stabilization plans.
  • Navigate complex service systems to reduce barriers to housing stability.
  • Facilitate timely referrals to community providers and behavioral health services.
  • Deliver services with trauma-informed, person-centered approach.
