Job summary

SOAR Community Services in Brantford, ON is seeking a Housing Resource Centre Case Manager to support individuals experiencing homelessness and at-risk families. You will deliver direct client service, complete SPDAT assessments, and coordinate housing applications with Brant Access to Housing, Canada-Ontario Housing Benefit, and related funds.

Qualifications

• College diploma in Human/Social Services or equivalent.
• Minimum 2 years’ combined placement or paid experience with at-risk individuals or homelessness.
• Proficient with MS Office and familiar with databases; valid driver’s license.

Responsibilities

• Deliver direct client service to individuals and families at risk of homelessness.
• Complete SPDAT assessments and support housing applications (Brant Access to Housing, Canada-Ontario Housing Benefit, Housing Stability Fund).
• Collaborate with Housing Services to ensure program delivery.
• Apply a harm reduction approach in service delivery.
• Maintain documentation in multiple electronic databases for reporting.
• Participate in department and sector case management meetings.
• Liaise with landlords and referral agencies based on client priorities.
