Qualifications

• Minimum three years related experience in direct service delivery to adults with serious mental illness.
• Demonstrated experience supporting individuals with mental health and substance use issues.
• Valid Driver's license and access to a personal vehicle.

Responsibilities

• Conduct needs assessments and support individuals with complex mental health needs.
• Develop and implement care plans with monitoring until clients are discharged.
• Respond to crisis calls and collaborate with various stakeholders.
