Job summary

Phoenix Residential Society in Regina is seeking a full-time Intensive Case Manager to provide person-centered supports, referrals, advocacy, and system navigation for individuals who were chronically homeless. You will work as part of HOMES to coordinate services and help participants access housing, health, and supports.

The role requires a university degree in human services and at least two years of homelessness/mental health experience, plus a valid Class 5 driver's license and reliable

Qualifications

  • University degree in human services field (Social Work, Human Justice, Psychiatric Nursing, Chemical Dependency).
  • Minimum of two years experience working in homelessness, mental health, and addictions.
  • Strong knowledge of Housing First framework.
  • Knowledge of community resources for homeless individuals including income supports, life skills, health, and addictions services.
  • Understanding of addictions models including stages of change and harm reduction.
  • Ability to work independently and as part of a team.
  • Proficiency in maintaining a non-judgmental trauma-informed approach.
  • Strong organizational skills and ability to manage multiple tasks.

Responsibilities

Phoenix Residential Society, a community based health care organization, has an opening for a full time position as Intensive Case Manager in our Housing and other Management and Engagement Services (HOMES). The Intensive Case Manager position is an integral part of a team-based approach in the provision of support services, referrals, advocacy, and system navigation to those individuals who were chronically homeless. Using a person-centred approach, provide case management services for a caseload of up to 12 participants.

Qualifications:
  • University degree in human services field (Social Work, Human Justice, Psychiatric Nursing, Chemical Dependency)
  • Minimum of two years experience working in the field of homelessness, mental health, and addictions.
  • Must possess as strong working knowledge of the Housing First framework.
  • Must possess a strong knowledge and understand of systems, supports, and available community resources for the successful delivery of services for persons that are chronically or episodically homeless including income supports, life skills support, health, mental health, and addictions services.
  • Must possess a strong working knowledge of addictions models including stages of change and harm reduction
  • Ability to work independently and as a team
  • Must demonstrate a proactive, hands-on whatever it takes approach to the provision of supports and services
  • Must utilize a non-judgmental therapeutic trauma-informed care approach at all times.
  • Strong organizational skills and ability to prioritize and manage multiple tasks simultaneously.
  • Must possess valid Class 5 drivers license and have daily access to a reliable vehicle to transport residents and execute daily duties
  • Criminal record check required upon hire.
  • Must have valid First Aid/CPR certification.
