Job summary

Stonehenge Therapeutic Community seeks two Intensive Case Managers to work across Guelph-Wellington and Wellington County, primarily based in Mount Forest. You will conduct intake, assess needs, coordinate housing entitlements, and link clients to health and social services with partner agencies.

Some evenings and weekend rotation apply. Qualifications include a health/social services degree, at least one year in addictions/mental health, and strong documentation skills.

Qualifications

  • College Diploma or University Degree in mental health, addictions, or social services.
  • Minimum one year of progressive experience in addictions or mental health; trauma-informed practice is an asset.
  • Experience in social services with working knowledge of case management systems and planning techniques.
  • Previous experience in housing is preferred.
  • CSC Clearance conducted by PWGS Canada.
  • Standard First Aid/CPR, Level C.
  • Valid G Driver's License (G2 or G) and satisfactory motor vehicle record.
  • Reliable vehicle and willingness to transport program participants.
  • Proof from insurer of at least $2 million general liability and acknowledgement of vehicle use scope required.

Responsibilities

  • Intake and assessment
  • Process & accept referrals from community partners
  • Obtain all appropriate consents & authorizations for clients' participation and effective communication with service providers
  • Conduct intake and enrollment activities, including gathering eligibility documents
  • Refer and link ineligible referrals to appropriate services
  • Process program discharges
  • Assist clients with move-in and entitlements access
  • Create and support implementation of individualized care plans with health entitlements and life skills
  • Create and implement crisis response plans and educate on crisis intervention services
  • Home visits and support meetings at least monthly
  • Coordinate with CMHAWW and other partner agencies and stakeholders
  • Provide substance use expertise and consultation to partner agency staff
  • Participate in program development activities
