Job summary

The Case Manager at Trellis Society for Community Impact supports clients in finding and maintaining housing within a harm reduction framework, focusing on youth in the community of their choice.

They manage a caseload, address housing, basic needs, medical appointments, and connect clients with mental health services, addiction counseling, and system supports while advocating for family reunification and community resources.

Qualifications

• Diploma or degree in a human services field; equivalents considered.
• 2-5 years of experience with individuals and/or families using a strengths-based and trauma-informed lens.
• Knowledge of brief therapeutic strategies, Housing First, harm reduction, crisis management, and Indigenous social work practices is an asset.
• Valid driver’s license and reliable vehicle; Monday to Friday with flexibility for evenings/weekends.
• Current Standard First Aid CPR-C, ASIST, and TCI considered an asset.
• Able to obtain clear Criminal Check with Vulnerable Sector Search and Child Welfare Intervention Check.

Responsibilities

• Engage clients and their natural supports with empathy and consistency.
• Support clients in goal planning using client-centered, strength-based approaches.
• Assist with housing navigation, life skills development, budgeting, and employment opportunities.
• Build safe relationships, set boundaries, and manage crises as needed.
• Collaborate with housing liaison and community partners; ensure timely documentation.
• Advocate for family reunification and navigation of needed services.
