QUALIFICATIONS
  • Post‑secondary education with a focus on Social Services or Developmental Services is required. Knowledge of health‑care procedures and mechanical equipment – wheelchairs, lifts, G‑tubes and other specialized equipment – is required.
  • Post‑secondary education in a non‑related field requires a minimum of 1 year of related work experience.
  • Successful completion of internal legislated and mandated training is required.
  • Valid driver’s license is an asset.
JOB SUMMARY

Identifies the needs of individuals supported and delivers supports and programs consistent with life plans to enhance their capacity to function in all aspects of community living.

RESPONSIBILITIES
  • Identifies program needs of individuals supported, participates in development and implementation of life plans, assists in monitoring achievement of personal outcomes.
  • Provides case management to individuals on caseload, including appropriate personal and physical care, medication administration, crisis response, advocacy, liaising with family, medical and other professionals and service providers to address needs.
  • Devises and provides life‑skills training and emotional support on a one‑on‑one or group basis and refers to professionals when necessary.
  • Maintains documentation, monitors finances, ensures an appropriate standard of living, and assists with locating housing for individuals supported.
  • Provides direction to part‑time staff as required.
