Clinical Service Hub Team Lead

Overview
Reporting to the Chief Executive Officer, the Clinical Service Hub Team Lead is responsible for overseeing the clinical services that will operate within the Kenora Emergency Shelter. These clinical services will be both internal to the Canadian Mental Health Association Kenora Branch and external partnerships established through formalized Memorandums of Understanding.
The Clinical Service Hub Team Lead will coordinate symptom management, life skills teaching, supportive counselling, family support, crisis support, and the coordination of these services to patrons utilizing the Kenora Emergency Shelter. Additionally, the role will develop relationships with external providers, engage in system advocacy, and provide education and consultation to service hub staff. The position will oversee the service planning process for patrons within the Kenora Emergency Shelter and assist in program planning and evaluation.
Responsibilities
- Coordinate clinical services within the Kenora Emergency Shelter and with external partners as defined by MOUs.
- Coordinate symptom management, life skills teaching, supportive counselling, family support, crisis support, and related service delivery to shelter patrons.
- Develop relationships with external providers and engage in system advocacy; provide education and consultation to service hub staff.
- Oversee the service planning process and assist in program planning and evaluation.
- Participate in program development, monitoring, and evaluation activities.
- Engage in ongoing collaboration with community partners to support recovery planning for clients.
- Liaise with key community stakeholders and partners to assess community and client needs.
- Ensure safety and quality standards in service delivery within the shelter context.
Qualifications
- BSW, MSW or registration with the College of Nurses of Ontario, or eligible for registration with a regulatory College.
- Experience working with individuals who are homeless and marginalized with mental health and addiction challenges, or demonstrated ability to work with this population.
- Empathy, understanding, patience, and strong interpersonal communication skills.
- Knowledge of community resources and ability to communicate professionally with community agencies.
- Cultural competence for diverse populations; understanding and applied knowledge of Recovery Model.
- Understanding of trauma-informed practice and implementation; good documentation skills.
- Ability to learn and understand online scheduling; proficient computer skills.
- Ability to manage conflict and crisis situations; leadership and supervisory experience; time management skills.
- Ability to work independently as well as in a team; creative problem solving.
- First aid & CPR / AED, Mental health first aid and NVCI or willingness to obtain.
Duties
- Adherence to all policies and procedures of Canadian Mental Health Association Kenora Branch.
- Screening of patrons using approved clinical assessment tools (VI-SPDAT, OCAN, etc.).
- Ensure diversion of patrons to clinical services through the Kenora Emergency Shelter Operations Team Lead.
- Develop individualized recovery planning and strategies to support goal achievement.
- Engage with clients to connect them to services and supports.
- Make appropriate recommendations for supports and services.
- Consult and collaborate with community partners to support recovery planning for clients.
- Liaise with key community stakeholders to assess needs.
- Maintain working linkages with law enforcement, primary care, human service agencies and informal community resources.
- Conduct risk assessments and continuous evaluation of client housing needs and satisfaction.
- Advocate for clients and link to services to meet recovery goals.
- Provide ongoing client satisfaction evaluation and reassessment of service requirements in consultation with the client.
- Direct provision of side-by-side support and provision of supportive counselling as appropriate.
- Model problem solving and priority setting; refer to community agencies as needed.
- Provide service navigation to clients; maintain client files per policy; monitor outcomes and evaluation.
- Represent CMHA Kenora Branch professionally; draft correspondence and reports; participate in administrative tasks and office maintenance.
- Engage in educational and professional development opportunities; ensure safety of patrons during shifts and adherence to WHS regulations, infection control, WHMIS, and Safe Work Procedures.
- Contribute to a safe environment and support an organization that avoids and corrects actions that may harm service delivery.
- Assist with self-care and perform other duties as directed.
